VET is a collective of VCSE organisations working together with a specific focus on developing and delivering health, care, and wellbeing services in Lincolnshire working with partner agencies including the NHS and Local Authorities in Lincolnshire.  VET is going through an unprecedented and exciting time of growth and development.  To support this development, we are recruiting to this key role for the development and delivery of:

  1. Building a trusting relationship with the Lincolnshire Primary Care Network [PCN] Alliance, identifying and realising opportunities for developments within social prescribing, including expanding geographic reach, adding additional services, or broadening target groups.


  1. Leading on the development and delivery of NHSE/I Partnership Agreement for Social Prescribing – developing a shared local plan for social prescribing using Population Health Management data and partnership working with commissioners, Local Authorities and VCSE organisations, understanding how this information could support improving health inequalities through social prescribing.


  1. Work in collaboration with our social prescribing providers, key partners, and people with lived experience to transition from the current social prescribing model into Lincolnshire’s ambition, through co-design, use of business intelligence, exploring opportunities to bring together current social prescribing schemes and understand the synergies, opportunities and added value of integrating ways of working.

For this post applicants will have: a commitment to addressing health inequalities and have demonstrable experience of developing local Social Prescribing schemes working with partner agencies;  excellent project management, delivery, and evaluation skills; highly developed communication and engagement skills which sustain effective working relationships across a broad range of partners; and experience of developing new partnerships across health, care & wellbeing services and a person-centred approach. Applicants should also hold a degree level [L6] qualification and have over 3 years’ experience in health, care, and wellbeing services either in the VCSE or statutory sectors.
