VCS and LCVS are looking for several Social Prescribing Link Workers to join their teams. Link Workers will be part of a multi-disciplinary team either within a local Primary Care Network to liaise with GPs, or as part of a local NHS Mental Health Transformation team.
Roles are available in Lincoln North and Stamford.

We are looking for organised, self-motivated, and confident individuals to join our established Social Prescribing teams working with adults with complex health and mental health needs helping them to access the support they need to live independent and healthier lives.
Positions are available across Lincolnshire and applicants must have experience of working with adults and have a background in health and/or social care or experience of supporting people’s wellbeing in a community setting.

You must be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing social and emotional challenges as well as have great interpersonal skills and ability to support people, community groups and local organisations.

You will work alongside multiple GP practices and Mental Health Transformation project sites independently managing a caseload and new referrals, providing personalised support, and connecting patients with community support and services as well as health care providers to improve independent living, health and wellbeing.

It is essential that you have an understanding of how to prioritise need, be able to recognise when to refer on to specialist services, being confident in your ability to communicate at different levels with people from all backgrounds and across both the health and social care sectors. You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
