Social Prescribing Lead

Lincolnshire Community and Voluntary Services (LCVS) are looking to recruit a lead for our Social Prescribing team.

Social prescribing can help to strengthen community and personal resilience. It reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. Link Workers support people who have been referred to the service either through self-referral, by GP’s, or from community and health partners.

The post holder will be a senior manager within the LCVS Senior Management team and strategically leading a team of Social Prescribing Link Workers to provide an effective service and contribute to the strategic development of Lincolnshire’s social prescribing offer.

You will work closely with primary and social care services to

  • Support person-centred integrated care navigation,
  • Develop social prescribing referral pathways
  • Build LCVS credibility with partners to raise confidence and awareness in the services we provide
  • Position the organisation appropriate to maximise future opportunities.

You must be able to ‘hit the ground running’ and be able to manage multiple priorities effectively within a fast-moving environment.

You will work within the third sector, alongside colleagues from health and social care in a vital developmental role to develop systems that support and enable the smooth and consistent running of the service.

You will build trusted relationships with people at all levels across the Primary Care Network, community groups and Voluntary Community and Social Enterprises (VCSE) organisations and services as well as work with partners to provide support to organisations involved in social prescribing.

If you have extensive senior level experience in delivery of person-centred services in a health, social or community setting; previous experience of managing staff and volunteers, we would love to hear from you.
